Stethoscopes and Dog Tags: Help, Please!

Okay, so I made a button, but the problem is I don't know how to get the grab my button thing where it has the code underneath. Help anyone?!

Oh and how do y'all like my button? I need y'alls opinion! I spent forever on it but I'm not sure, I got tired of doing it so I just settled....

I hope there are some computer geniuses out there, cause I am lacking in that department.
